The Anchor Stroke Rehabilitation Program provides our patients with a coordinated, multidisciplinary team led by rehabilitation specialists that include therapists and nurses committed to meeting the unique needs of home care patients and their families. 

WHAT WILL THE ANCHOR STROKE REHABILITATION PROGRAM PROVIDE? 

. An on-site patient needs assessment conducted by a registered nurse 

• A multidisciplinary clinical approach that allows the patient to: 

Achieve maximum functional mobility 

• Maintain an activity level as near to normal as possible 

• Avoid symptom recurrence 

• Financial outcomes that include: 

• Decreased number of days in acute, subacute and skilled nursing facilities 

• Decreased utilization of emergency and urgent care centers 

• Decreased overall cost of stroke management 

• Individualized, physician directed plan of care that includes: 

. Patient/family education 

Home exercise programs 

• Supplemental educational materials 

• Community resource identification and linkage 

• An outcome measurement component
