How the program works:
An Anchor nurse performs a free in-home client assessment. The Anchor nurse consults with the physician to determine the client's individual home care needs. A detailed plan of care is developed based on the diagnosis, care needs, family support, prognosis, home environment and community resources available. The best team of Anchor health care professionals is selected to implement the total home care program. The Anchor nurse makes scheduled visits to the home to monitor treatment, review and/or revise the care plan.

Ventilator and Tracheotomy Care
High Tech Services such as: Iv and antibiotic therapy, Internal nutrition, and pain management
Routine Home Care such as: Wound, Diabetic and Colostomy care
HIV/AIDS Care
Care of Trauma Victims
High Risk Pregnancy Care
Rehabilitation Services
Social Work Services
Nutritional Counseling
Respit Care/Child Care
Assistance with Activities of Daily Living
Services Provided by:
Nurses: Registered, Licensed Practical/Vocational, IV Certified, Pediatric Nurse Specialists
Therapists: Physical, Occupational, Speech
Medical Social Workers
Certified Home Health Aides
Homemakers
Companions/Sitters
Live-Ins
