A detailed plan of care will be developed based on the diagnosis, care needs, family support, prognosis,home environment, and community resources available. How to Make  Referral 
Call our Intake Coordinator at 219-263-0411 and be prepared to offer information regarding the potential patient's demographics, insurance policy numbers and contacts, physician name and orders, and services requested. If home care is not covered by a patients insurance, we have private pay rates available. We will strive to meet all of your needs and requests in a timely and professional manner.
